Our New Digital Front Door - Helping You Access the Right Support
Nuneaton and Bedworth Primary Care Network are thrilled to announce the launch of a new digital platform that will help connect people across Nuneaton and Bedworth, and our wider community with the health, wellbeing and community support they need. Developed in partnership with RIVIAM, the new system supports our Personalised Care Team and strengthens the way we coordinate care across local services.
The platform provides our team with a modern, secure way of managing referrals, coordinating support and keeping track of the outcomes that support patient care. By bringing everything together in one place, it helps us work more closely with GP practices, community services, local authorities and voluntary organisations to provide more joined-up approach.
Introducing Our New Services Directory
As part of the launch, we have also introduced a new online Services Directory that acts as a digital front door to local health, care and wellbeing support.
Whether you are looking for support for yourself, a family member or someone you care for, the directory makes it easy to find local services all in one place. Instead of searching multiple websites, you can access hundreds of community, health and support services through a single search.
The directory includes information on:
