At Nuneaton and Bedworth Primary Care Network (PCN), under the leadership of our Health Inequalities Lead and Non-Clinical Director Lynn Slater, we have been committed to building strong relationships with key partners, including local councils, George Eliot Hospital, and various community organisations. By aligning efforts and leveraging existing initiatives, we aim to address healthcare barriers and reduce inequalities in our region. Our collaboration spans several key partners:
- George Eliot Hospital: Engaging with specialised teams for screenings including targeted lung health checks, cancer screening and community health checks.
- Borough and County Councils: Working closely with community teams on veterans’ health and wellbeing initiatives and our different communities and their differing needs.
- Coventry & Warwickshire Partnership Trust (CWPT): Supporting inclusion networks and addressing mental health needs.
- HealthWatch: Conducting surveys and gathering vital insights on community healthcare barriers.
- Justice Services: Piloting innovative programmes for individuals impacted by socio-economic, financial, immigration and judicial challenges.
Tackling Inequalities Through Core20PLUS5
The Core20PLUS5 framework guides our work in addressing healthcare gaps, focusing on:
- Maternity care for underserved populations.
- Mental health with annual physical health checks for severe mental illness (SMI) patients.
- Improved management of chronic respiratory diseases.
- Early cancer detection through community education and screening.
- Hypertension and lipid management to reduce cardiovascular risks.
Community-Centred Engagement
Our team has delivered targeted initiatives, including:
- Men's Health Events: Held at venues like the Gurkha Corner, these sessions provide health checks and cancer screening awareness.
- Support for Women on Probation: Events focusing on missed cancer screenings and general women’s health issues such as menopause.
- Veterans' Health Programmes: Conducting surveys and offering health checks tailored to veteran needs.
- Boaters' Health: Addressing inequalities for continuous cruisers through health assessments at marinas.
Collaboration and Inclusion
We’ve partnered with diverse community groups, including faith leaders from Sikh, Muslim, Hindu, and Nepalese communities, to co-create programmes that resonate culturally. Efforts to reach historically underserved groups, such as the Gypsy and Traveller communities and the farming population, are ongoing.
A Commitment to Evolving Health Initiatives
Our journey is far from over. We continue to identify and address gaps, build trust, and connect with more communities, including African, Jamaican, Chinese, and Eastern European populations. By maintaining close ties with our partners and the public, we aim to make healthcare equitable and accessible for all.
